CareGivers America is a regional provider of Non-Medical Home Care, Home Health, Medical Equipment, Medical Staffing, and Private Duty Nursing Services. Presently we have 15 offices throughout Pennsylvania covering 25 plus counties.
CGA is recruiting for a FULL TIME Office of Developmental Programs Patient Liaison to be based in Dallas, PA
Attend participant ISP Meetings to engage in the development of patient outcomes & goals.
Assist in training employees/surrogates in proper documentation according to outcomes in the ISP.
Review daily logs and provide re-training to employees as needed
Train employees upon hire and annually
Train surrogates upon Start of Care and as needed
Assist in review, completion and re-training of Monthly Progress Notes
Complete all applicable certified investigations (training to be provided, as needed)
Complete certified investigation peer reviews
Assist in the completion of administrative duties, as needed
Assist in gathering data for Annual Provider Qualification
Attend Quality Council Meetings & actively participate in the provider network
Ensure employees are complying with proper documentation of services provided.
Ensure accurate and timely completion and submission of monthly progress notes.
- High school diploma
- Must possess positive, enthusiastic attitude, good people skills, and professional appearance
- Valid PA Driver's License and willing to travel with use of a company car
- Ability to work independently without constant supervision
- Experience working with the Behavioral Health/Intellectually Disabled Population
- Office of Developmental Programs experience- 1 year preferred
